Medical Forms

Our Pack must have medical forms on file for all participants (youth and adults) at all events. 

Medical forms are valid for one year unless changes warrant a new form sooner.

For events less than 72 hours

  • Applies to ALL participants—in basic Scouting activities such as local tours and weekend camping trips less than 72 hours in duration.

For events longer than 72 hours

A pre-participation physical is needed:

  • Long Term Camps (e.g. Goshen), Wood Badge, Jamborees
